You need advisor approval before registering for SOC 470 credits. Complete the fillable form and email to dpeters@iastate.edu.

For internship supervisors
To be awarded credit, your supervisor must submit a final evaluation form when the internship ends. Your supervisor may submit using the PDF or via the web portal. It is the students responsibility to provide the final evaluation forms or links to their internship supervisor.

HOWEVER, transition to Workday Student may require electronic forms.
